Perhaps it’s because many people have romantic and misplaced notions about nature. I am 81 years old next month. I have been in serious pain from Fibromyalgia since I was in my 50s. Certain facets of acute cannabinoid exposure, including tachycardia, hypothermia, orthostatic hypotension, dry mouth, ocular injection, intraocular pressure decreases, etc. are subject to rapid tachyphylaxis upon continued administration (Jones et al 1976). No dose tolerance to the therapeutic effects of Sativex has been observed in clinical trials in over 1500 patient-years of administration. Additionally, therapeutic efficacy has been sustained for several years in a wide variety of symptoms; SAFEX studies in MS and peripheral neuropathic pain, confirm that Sativex doses remain stable or even decreased after prolonged usage (Wade et al 2006), with maintenance of therapeutic benefit and even continued improvement.

In order to manage pain, we recommend ingesting full spectrum CBD oil daily in the form of Tinctures or Gel Capsules. I wanted to tell people here that CBD has been very effective for my anxiety, and helps with insomnia. Of course, because legal marijuana is in such a confusing transitional period, even here there are potential exceptions. The U.S. Food and Drug Administration (FDA) approved Epidiolex, a treatment for a rare form of pediatric epilepsy that contains CBD. The clinical trials performed with Sativex have recently been assessed in two independent review articles (Barnes 2006; Pérez 2006). In a Phase II clinical trial in 20 patients with neurogenic symptoms (Wade et al 2003), Tetranabinex, Nabidiolex, and Sativex were tested in a double-blind RCT vs placebo (Table 1). Significant improvement was seen with both Tetranabinex and Sativex on pain (especially neuropathic), but post-hoc analysis showed symptom control was best with Sativex (p < 0.0001), with less intoxication than with THC-predominant extract.

Hemp oil is an oil extracted from the hemp plant. All plants in the Cannabis genus can produce the oil, but usually only industrial hemp is used to make hemp oil. Industrial hemp is a hemp varietal which has been cultivated specifically for industrial production, and it has a minimum of the psychoactive substances associated with the genus, most notably THC. Hemp oil is typically almost free of THC, and it has no psychoactive properties.
CBD directly interacts with a number of proteins in the body and central nervous system, a few of which are components of the endogenous cannabinoid system. For instance, CBD binds to both the CB1 and CB2 cannabinoid receptors, but it binds in a way that sets off a reaction that is essentially the opposite of what THC does. CBD is an inverse agonist, while THC is an agonist at CB1. Simply put, CBD is not intoxicating; at the molecular level, it does the opposite of what THC does.
